Mr. John is a 65-year-old male diagnosed with Peripheral Arterial Disease (PAD). He is a retired construction worker who presents to the healthcare facility with complaints of severe pain in his right foot and recurrent foot ulcers. He has a medical history of hypertension, hyperlipidemia, and type 2 diabetes mellitus. He lives alone in a small apartment with limited mobility due to his condition. He is anxious and apprehensive about the prospect of losing his foot, fearing the impact it will have on his independence and quality of life. On assessment, Mr. John's vital signs were: BP 150/90 mmHg HR 80 bpm RR 18 breaths/min Temperature of 37°C O2 Sat 96% on RA Inspection of his right foot shows coolness to touch, pallor, and absent pedal pulses. Multiple foot ulcers are present, particularly over the dorsum and lateral aspect of the foot, indicating poor perfusion and compromised tissue viability. His pain is described as throbbing and exacerbated with movement. Based on clinical findings and diagnostic tests such as ankle-brachial index (ABI) measurement, duplex ultrasound, and angiography, Mr. John is diagnosed with advanced PAD necessitating surgical intervention. The proposed treatment plan involves below-the-knee amputation (BKA) of his right foot to alleviate.
