The patient is a 76-year-old Tom diagnosed with chronic obstructive pulmonary disease (COPD) 5 years ago. His wife died 8 months ago and Tom now lives alone. He is a former smoker and has not smoked for 5 years. The patient reports increasing oxygen consumption at home over the last 3 years and requires at least 2 1/min continuously over the last six months. Tom has lost a lot of weight over the last six months as preparing meals is very tiring for him and he quickly gets breathless. The patient complains of reduced mobility, claiming he has no strength and feels unsteady when walking. Tom spends most of his time sitting in a lounge chair watching television. The patient misses his wife
very much and feels lonely.
