The patient is a 13 year old female diagnosed with ADHD and DMDD, presented for a follow up evation with her aunt who serve as her care giver. The patient has recently faced school suspension stemming from behavioral conflict, including a physical altercations with other students and episodes of disrespect towards teacher. Patient reported she broke a school door, hence, she was suspended two days from the school. Patient denies fatigue, tirdnedness, irritability, SI, HI, AVH at this time. Patient medications where refilled and a follow up appointment scheduled for the next four weeks. Patient medications includes guanfacine 1mg qam, Depakote 500mg qhs, abilify 10mg daily, and Qelbree 200mg daily.
