Intervention and Evaluation Plan
Purpose
The purpose of this assignment is to assist students in developing an intervention and evaluation plan for their Education Practicum Project. Both intervention and evaluation are integrated in this paper to help students appreciate the importance of tying them together.
You may not implement your project until after your proposal has been approved by your instructor.

Narrative description (paragraph format) of your implementation plan for your evidence-based educational intervention.

Step-by-step format of how you will implement your quality improvement project

Provide a detailed teaching plan using the required Teaching Plan Template, to include:
At least four well-formulated objectives
Demonstrates appropriate level Bloom's taxonomy verbs for the learner
Include example of all teaching materials outlined in your Teaching Plan in the Appendix.
Evaluation
The evaluation plan should clearly describe the following:
Two methods for measuring outcomes that are consistent with the PICOT question.
Test of learner knowledge/skill and/or evaluation of learner confidence
Evaluation of the lesson content, educational intervention, and teaching effectiveness of the instructor.
Copy of each evaluation method (survey/quiz, etc.) in the Appendix
Revision Process
