At the weight check (7 days ago) JR had gained 7 ounces from his birth weight of 7.5 lbs (3.5 kg) and 0.5inches. At the weight check the LPN documentation states there was no indication of jaundice His length at birth was 20 inches (50cm) and HC 13.5 inches (34.5cm). The 2- day visit indicates that mom was breastfeeding and was an anxious 1st time mother. Her mother accompanied her to the visit. JR was at birthweight; breast feeding was observed and you documented good technique and appropriate bonding. JR is a 40 -week infant, non-eventful vaginal birth to a 36-year-old mom; G1-P1 Bilirubin at hospital discharge 12 mg/dL.
