Consider the following information:

Nursing Diagnosis: Increased risk of a new HIV diagnosis among Hispanic males in San Diego County related to male-to-male sexual contact, lower level of high school graduation, and high numbers of uninsured individuals as evidenced by a high rate of AIDS-related deaths at 487.2 per 100,000.

Long Term Goal: Reduce the rate of newly diagnosed HIV cases among Hispanic males in San Diego County by 25% by the end of June in 2024.

Short Term Goal: Provide specialized education on HIV/AIDS prevention and risk factors to at least 30% of the Hispanic male population in San Diego County within the next six months.

Target Population: A classroom of Hispanic males between the ages of 18 and 21 in San Diego County

Develop a community based educational plan on student identified goals while meeting the health literacy needs of the community

  • Formulate 3 measurable objectives. There are three components of measurable community based learning objectives: An action verb, a specific behavior that is performed by the community members, and a specific timeline for accomplishing the behavior.
  • Include content for each objective. It should include the specific content you would provide to help the aggregate accomplish the objective.
  • Include a teaching strategy for each objective based on social learning theory. Be sure the method of instruction relates to social learning theory and the aggregate.
  • Indicate how long you would take to provide the content information.
  • Indicate how you would determine whether the objectives were met (Q/A session, pre/posttest, gaming, and demonstration/return demonstration).
  • Describe the literacy levels of your target population based on age, gender and estimated levels of education. Identify 3 ways you would develop educational materials that best fit the literacy levels of your aggregate.
