Question
Chapa is a 20-month-old child who lives on a reservation in Minnesota with his parents and 4 brothers and sisters (ages 8, 7, 6, and 3). Chapa was recently referred by the Well-Child Services of the Public Health Department to an interdisciplinary team in the Early Education program (Early Childhood Special Education Teacher, OT, PT, speech pathologist) for an initial developmental evaluation at his School. Previous medical concerns include a history of ongoing seizures currently controlled with medication), spastic quadriplegia, general developmental delay, feeding problems (currently bottle fed and eats Stage 2 purees), and esotropia/amblyopia/strabismus. He was born at 30 weeks gestation with a birth weight of 2 lbs. 3 oz. and spent his first 4 months in the NICU at Children's Hospital in Maple Grove. At 8 months of age, Chapa had infantile spasms and was brought to a district clinic on the reservation. Chapa's left eye has been recently patched for several weeks to try to keep his right eye from turning in to midline.
The family lives in a 2 bedroom rambler. Chapa does not crawl, but can sit with the support of a pillow for short periods of time. His father works part time at the reservations general store. His mother takes care of children for other families in Chapa's family home and is involved in demonstrations of beadwork at the reservation's museum. The 3 older siblings are at school during the day.
