Case study:

On your clinical placement in an aged care facility, your preceptor asks you to assist with the Activities of Daily Living (ADLs).

Your preceptor is caring for Mr Jones, a 70-year-old male who returned from hospital last week post treatment of a chest infection. Prior to hospital admission, Mr Jones had been managing his ADLs with minimal assistance. He has a history of back pain, which contributes to poor mobility.

You visit the patient and notice a foul smell in the room. Mr Jones says he hasn't been able to keep up with his ADLs due to new back pain. He accepts your assistance.

While assisting with the ADLs, you observe broken skin with ooze on the sacrum region (Stage 2 pressure injury). You report this to your preceptor. Your preceptor discusses this with the In-Charge Nurse and organises an educational in-service to promote awareness of pressure injuries. Your role is to assist your preceptor in designing and developing an evidence-based educational plan in the form of an interactive poster presentation.
