At 15 hours of life he is no longer interested in feeding and appears dusky. His respiratory rate is 65 breaths/min, his oxygen saturation on pulse oximetry is 80%, and his capillary refill is 3 seconds. No heart murmur is audible, but a loud single second heat sound is noted.
