Question

Hospital readmissions, particularly within 30 days of discharge, present significant challenges by increasing healthcare costs and suggesting potential deficiencies in patient care and discharge planning. These readmissions can negatively impact patient outcomes, disrupt recovery, and burden healthcare resources. This project assesses the effectiveness of a structured transitional care program in reducing 30-day readmission rates, guided by the PIO framework: P (adults with chronic conditions post-discharge), I (structured transitional care), and O (reduce 30-day readmissions). The theoretical framework used is the transitional care model, which is grounded in an integrated, patient-centered care approach, emphasizing the importance of multidisciplinary collaboration. A quasi-experimental design is utilized to systematically analyze the effectiveness of transitional care interventions, such as patient education, medication reconciliation, and follow-up care. Data will be collected from hospital records to identify 30-day readmission rates before and after implementing the transitional care program. Statistical methods will be employed to analyze changes in readmission rates, focusing on whether the intervention significantly impacts reducing readmissions. Insights gained will be used to reallocate resources toward preventive care and promote long-term health improvements, ultimately benefiting healthcare systems and patient care.
