You are caring for Mrs. Ray, a 92 year old admitted with decompensated heart failure to telemetry unit. She lives at home with her son and has a history of dementia. During your morning assessment, Mrs. Ray is confused to person place and time. She is pushing your hands away during some aspects of your morning care. She had an intermittent infusion device in place and has not attempted to pull at it. Her vital signs are BP 158/90, HR 126, RR 22 and SpO2 of 95%. At 9am you are administering her prescribed medications: metoprolol 25mg po, furosemide 40mg po and potassium chloride 20mEq po. Mrs. Ray is refusing to take her medications.
