A tool that I have found to be especially helpful in everyday practice is the Braden Scale. It provides an easy checklist that requires six quick questions that when answered in accordance with each patient's individual condition. The scale is broken down into 6 categories. Each category is then broken even farther down to take into account each patient's condition. The lower the score the greater risk there is for skin injury. Often it is built into whichever charting system that is being used. This makes documentation very simple because the score populates into your charting automatically. In Labor and Delivery we very rarely have patients that would score anything less than the highest score. This is due to the fact that most women are up active and healthy in child bearing years. They also pay more attention to consuming a healthier diet. While the Braden Scale encompasses all patients it does allow for complacency when dealing with a generally healthy patient population. When treating a more diverse patient population the Braden Scale may prove more beneficial.
