Case Information
A 46-year-old male injured his right wrist yesterday playing basketball. The patient has a small nondisplaced fracture evident on X-ray. The patient is complaining of significant throbbing pain.
1. What opioid medication would you choose? Why?
2. What questions should you ask about his history before prescribing that medication, and why are you asking those questions?
3. What is the maximum number of days or pills you would prescribe?
4. What patient instructions should you give him about the medication? Would you suggest any other OTC medications for him while he's taking this prescription?
5. I would suggest that the patient could also take NSAIDs or Tylenol for supplemental pain control. If the patient had only a wrist sprain instead of a wrist fracture would you choose a different medication class? If so, what and why?
6. If the patient did not have a wrist injury, but had a back strain with significant muscle spasms: What two drug classes (that we have covered in Weeks 1 or 2) could you recommend instead of opioids?
7. What muscle relaxer are you most likely to prescribe? Why?
