Mr. Smith is a 69-year-old man that was seen 3-weeks-ago for a gash on his heel that was cultured and showed an infection. He had been scheduled for knee replacement surgery until this occurred and the orthopedic doctor prescribed an antibiotic for 14 days and the surgery was scheduled the day after the antibiotics were to be completed. He now has arrived at the emergency room unable to put weight on the leg and walk. He is a full code. His foot, lower leg and knee are red, extremely swollen, and warm to touch. The wound on the heel is open and leaking purulent drainage. The patient states his entire leg hurts and rates his pain a 10/10. He took the antibiotics as prescribed and his last dose is tomorrow.
