Part A - profile and questions :

1. You must create a realistic profile of a disability client who is seeking a case-plan (to be used when you act as a disability client in the role-play).

2. You must develop questions to be asked during a first case-plan meeting with a client, with the aim to build a clear and comprehensive picture of the client's background, environment, support system, needs and wants. The needs of the family and or carers should also be considered.

Part B - role-play :

1. The questions developed in Part A ii) will be used in a role-play with a 'disability client' understand the needs and wants of the client and to refer the client (and their family and/or carer) to various community services where their needs (and wants where possible) can best be met.

2. When you are playing the 'case-manager' will take notes (to be submitted in case-plan format) whilst interviewing the client.
