Patient is unable to drive. She is an ex-smoker for 15 years, quit in her 40s. Patient lives in a raised ranch. Nursing notes: Patient alert & oriented, speech slow, edema of legs, HTN, cough, no known allergies Vitals: T = 97°, BP = 176/92 mmHg, Height = 5'5", Weight = 291 lbs., HR = 68 bpm resting, 96 bpm with mobility to W/C Medications: Lisinopril, Carvedilol, Lasix, Metformin Interventions by other professionals: Occupational Therapy, Nurtrition Physical Therapy Examination S: Patient c/o pain in B knees due to OA, rating it as 3/10 with ambulation. Patient c/o dyspnea with activity (walking). She would like to return home with her husband. Patient states that she is legally blind in Left eye but does have some vision in that eye, although blurred.
Patient is an obese female; pleasant and cooperative.
