Examine and describe the process you would follow to assess the organization where you plan to conduct your project for opportunities for improvement, including the identification of stakeholders. My DNP practice change project involves implementing a self-care diabetes management program to improve blood glucose levels in adults with type 2 diabetes in an acute care setting. The evidence-based intervention commonly includes structured self-care programs, focusing on self-monitoring of blood glucose, dietary education, physical activity promotion, and medication management (Powers et al., 2020). To assess an organization for improvement opportunities in diabetes self-management and educational will begin with a thorough organizational analysis and review of current programs, policies, and practices. Understand the organization's mission, goals, and past performance data to establish a baseline for diabetes education. Next, engage key stakeholders, including healthcare providers, administrative staff, educators, and patients. Use interviews, surveys, and focus groups to gather qualitative data on current programs' strengths and weaknesses. Stakeholder insights will help identify gaps for improvement. After collecting feedback, analyze the data to find specific improvement opportunities, I will prioritize recommendations and create an implementation plan with stakeholder involvement to ensure alignment.
