Outcome Evaluation Outcome evaluation is concerned with changes in health status; this may include morbidity, mortality, disability, and changes in potential years of life in the priority population. This type of evaluation is time consuming, expensive and changes usually cannot be seen in less than three to five years. Due to the extensive nature of outcome evaluation, we can develop an evaluation design and plan, but we cannot implement the plan within the time frame of an academic semester. For this phase, you will identify the evaluation design that will be used for the intervention, describe how and where the baseline data that will be obtained (depending on design, who is the control group, how were they identified, and similar qualities), and explain the metrics that will be utilized for assessment of outcomes as a result of the program. You should also include an overall outcome evaluation objective that will be used for measurement target population is 35-52 with hypertension.
