A 45-year-old male presents to the Emergency Department with the complaint of severe epigastric pain that radiates to the upper back and scapula area. He has had several episodes of acute pancreatitis over the last 5 years and reports that this is almost as severe as his last episodes. He has a 25-year history of alcohol abuse and reports that he feels as if he is dehydrated. On physical exam, he is tender over the epigastric area with decreased bowel sounds, he has a Cullen's sign and his vital signs are as follows: BP 92/64, HR 112, RR 24 and Temperature of 99.8 orally. He has mildly elevated levels of amylase and lipase. A plain film of the abdomen shows calcifications in the pancreas. How often do chronic drinkers develop pancreatitis and what is the mechanism of alcohol-induced acute pancreatitis? Describe one early and one late complication of acute pancreatitis.
