History of Present Illness: 75 year old Black male presents with complaint of increased weakness and debility x 1-2 weeks with associated symptoms of dizziness, numbness and tingling in extremities as well as mood changes, denies bleeding or any other associated symptoms. Reports he has tried some energy drinks which did not help but makes him stay awake at night, stated he is usually very active but not recently, blood pressure stable today. Vitamin B12 deficiency, Anemia, Hypotension, electrolyte imbalance, dehydration, and thyroid disease implicated in differential diagnosis.
