Question 

Write an Initial PSYCH VISIT, chief complain Psychiatric Evaluation/ diagnosis, Psychiatric visit HPI, social, screening tools and results, current Psych meds, MSE, Family History, diagnosis and recommended Plan for a 77years old female with is currently residing at Life care of Cottesmore, a skilled facility. She has history of Anorexia, depression and unspecified dementia. During the assessment, I have been tired for a long time. I iron is very low and she was recently started on iron 235mg on 3/12. Patient said I like sleeping till 11am when I am home, but here, they do not let you sleep till that time. I like when I sleep at least for 10hours. Today mym mood is better and I am trying to organize my closet. I just want to go back to bed. My appetite is zero and I cannot eat without throwing up. I am also taking antinausea medications. My kids are trying to get me food like cottage cheese but I can only eat a little. I have been in this facility now for two weeks and I cannot wait to go home. I have 3 kids, married and have 4 grandchildren. Current meds Bupriopion hcl 150mg 24hr for depression Melatonin 3mg PRN for insomnia. Patient ok to start Mirtazapine 7.5mg daily at HS to help with appetite. Called daughter too who is responsible for her care and she was ok starting Mirtazapine 7.5mg daily.
