PDSA Case Study – Small Test of Change

The leadership team at Strawberry Fields Hospital has identified the telemetry unit as a pilot unit to test strategies for reducing patient falls with injury. During a PDSA planning meeting, the nursing staff believe that the root cause of falls on this floor is medication induced and that changing the process for starting patients on new medications should be explored. Based on this input, the team decides to work on improving awareness and communication among the nursing staff (particularly during shift changes) when a medication change increases a patient’s fall risk.

They plan to test the following process changes:

• When a patient starts a new medication that can be fall-inducing, a red sticker reading “FALL ALERT – MED CHANGE” will be placed on the patient’s white board.

• An fall alert will be added to the patient’s Medicine Administration Record (MAR) in his/her medical record.

• At change-of-shift, nurses will review the patient’s fall risk assessment and the whiteboard at the bedside.

The pilot will be conducted for 3 days with the day shift. What data should be collected to determine the effectiveness of the pilot?
