You are the charge RN of the surgical floor and are responsible for calling in the coagulation studies for the patients on Warfarin (Coumadin). Mr. Hanson, 55 years old, is on Warfarin for chronic atrial fibrillation and has restarted his therapy following surgery. His HCP began his Coumadin therapy 3 days ago. He has been receiving 5 mg po daily since that time. Labs are as follows 2/11: INR- 1.4; 2/12: INR- 1.5; 2/13: INR- 1.6; 2/14: INR- 1.6. Use the SBAR template to call the HCP. Add any additional information that you would want to provide in your phone call.
