Question

A 59-year-old woman presented to her clinician with complaints of vaginal bleeding. She is post-menopausal and has not had a menstrual period in 15 years. Her physician scheduled her for an exploratory laparotomy with potential biopsy of the uterine lining. He ordered the usual pre-operative lab work which included a CBC, PT, aPTT and Type and Screen. Her Hgb was 7.9 g/dL so the physician wants to give her two units of packed cells before taking her to the O.R. He ordered the transfusion to be administered two weeks before her surgery. The blood bank received the properly labeled EDTA tube. The MLT checked her history and discovered that she h ad been transfused previously 35 years ago with 2 units of packed cells after excessive blood loss secondary to childbirth. The type and screen results were A Rh Positive with a negative antibody screen. The MLT crossmatched and issued two units of packed RBCs to the outpatient nursing unit. The patient was transfused without incident. On the day of surgery, the physician ordered a repeat CBC and Type and Screen. Her Hgb was 10.2 g/dL. The screen results were positive.
