Charlotte, North Carolina is my identified health community, and the diagnosis is Chronic Lower Respiratory Disease (CLRD); I compared it with the healthcare system in the Netherlands. One improvement based on systems integration would be implementing a regional health information exchange (HIE) platform that connects hospitals, clinics, and community health workers to streamline care coordination and ensure continuity of care for CLRD patients (Vest and Kash, 2016). To address the IOM Aim of being efficient, Charlotte's healthcare system could adopt automated patient triage tools that help prioritize care based on urgency and reduce unnecessary emergency department visits, which are common in respiratory-related illnesses (Powell et al., 2012). The Netherlands' healthcare system emphasizes strong primary care integration and continuity through shared electronic records and coordinated care teams. Charlotte could learn from this model by investing in interoperable systems and empowering primary care providers to serve as consistent care managers for chronic patients, leading to fewer hospital admissions.
