Question

The mother of a healthy 18 month old, who is in for his well child visit, is concerned when the Nurse Practitioner (NP) reviews the results of her child's autism screening.

The child's M-CHAT-R screening results were as follows. Parent answers no to the following questions:

Pretend to play - no

Use one finger to point - no

Does your child smile in response to your face or your smile? - no

All other answers to the M-CHAT-R were consistent with expected development. The NP assesses the 18-month-old: To get the toddler's attention, the NP says his name&points across the room at an interesting picture on the wall&says "Oh look, there's a picture of a dog with a balloon". The NP watches the child's face to see if he is interested in what the NP is pointing at.Result: toddler continues to play&stare at his pacifier, appearing disinterested.The NP then asks the mother the following questions:

Does your son bring you toys or favorite objects to share with, such as a book Answer: No;

When you say to your son "where is the light or plane" does he point to it? Answer: No;

The child's mother tells you that her sister has a child who is autistic and she is now worried her son may be autistic.She is also worried that she may have caused his autism by occasionally smoking marijuana (less than once a week) during the first trimester of pregnancy.

Identify three additional questions for the mother and/or health supervision assessments the NP can perform by utilizing the M-Chat-R screening tool and the developmental milestones identified for this age range in Bright Futures.
