Preoperative Diagnosis: Change in bowel habits and hematochezia. Postoperative Diagnosis: Two small polyps one in the ascending colon, one in the sigmoid cold snared off; internal hemorrhoids. Procedure Performed: Colonoscopy and Polypectomy Indication: This is a 56-year-old male who was recently noted in the last 2 years of increasing constipation and he has occasional hematochezia times 1 month. He did have one episode of some itching in the anal area. There are no risk factors for colon cancer. Preoperative Medications: Fentanyl 100 mcg IV; Verses 6mg IV Description of Procedure: The Pentax video colonoscope was inserted without difficulty to the cecum. The Ileocecal valve was identified. The appendiceal orifice was seen. The prep was fairly good. Careful inspection in the cecum, ascending colon reveal no lesions except for 2mm polyp which was cold snared off. Inspection of the hepatic flexure, transverse colon, splenic flexure, descending colon reveal no lesions. The sigmoid revealed a small 2mm polyp that was cold snared off. The rectum is normal. Retroflexion revealed minimal internal hemorrhoid. The patient tolerated the procedure well. Impression: Two small polyps in the ascending and sigmoid cold snared off. Internal hemorrhoids.
