Mrs. Burbank, 78 years old, is being discharged from the hospital today. She was admitted 7 days ago with a cerebral vascular accident that has left her with some residual right-sided weakness. When her husband Mr. Burbank leaves the room, she quickly pulls the nurse aside to say that if she has another stroke, she does not want tubes or machines and wants to "die with dignity". Mrs. Burbank has no written advance directives in her chart or otherwise. Mrs. Burbank begs the nurse not to discuss this with her husband since she does not want him to be worried about her condition. Mr. Burbank returns and asks what is going on.
