GH is a 35-year-old Caucasian female with a diagnosis of ADH combined type, MDD, and PTSD, who presented in the clinic for a f/u assessment and med management; she states she is currently on Therapy. Current meds include hydroxyzine 25mg daily, Wellbutrin ER 300 mg tab daily, and Concerta 36 mg tab daily. She reports everything is going well and has no concerns. She reports compliance with taking her meds and no changes in mood, sleep, and appetite. No side effects were reported from medications, denies suicidal ideations, hallucinations or hearing voices, smoking, or drug use. NP's plan of care is to continue current medication; medications refilled next follow-up in four weeks or sooner if any changes. Medications hydroxyzine 25mg daily, Wellbutrin ER 300 mg tab daily, Concerta 36 mg tab daily.
