While completing the pre-vaccination screening you note that Liam was born at 31 weeks gestation and was 1.8kg at birth.He does not have any other medical or surgical history. Liam's mother and father identify as being European. Liam's mumhas given informed consent for all vaccinations that are recommended for him as she wants him to be maximallyprotected.
