The patient is a 16 year old white female, with a past medical history of painful periods, who presents for complaints of burning/pain with urination and urinary urgency for 3 days. She states it started 3 days ago after having intercourse with her boyfriend and not peeing after. She goes 2 or 3 times an hour. She described it as a strong smell and has stomach cramping right above pelvis bone. States peeing makes the cramping worse. It started at the same time as her trouble peeing. She hasn't tried any treatments, and nothing makes it better or worse. Pain is moderate in severity. LMP 3.5 weeks ago, pt uses condoms and is on oral contraceptives. Denies any fevers, vaginal discharge or nausea/vomiting. On physical exam pt has tenderness to palpation in suprapubic region. VS 98.7 temp, 75 pulse, BP 103/72 and 105/75, 16 RR. Past medical history is painful periods, no surgeries or allergies. Became sexually active at 15, with one partner. What laboratory diagnostic test should be done for this patient? and what are the differential diagnosis?
