Question

CQI is a vital part of nursing and plays a big role in improving patient outcomes. When I worked as a CNA, I noticed that shift reports between CNA's and nurses sometimes lacked important information, which could affect patient care. "Effective nurse hand-off is essential for ensuring continuity of care and minimizing the risk of errors during patient transitions between shifts or units." (Galatzan, 2025, p. 43) To address this, I would suggest introducing a structured hand-off tool to make sure critical details are consistently communicated. A helpful method to support this would be the Plan-Do-Study-Act (PDSA) cycle. First, we could plan the checklist, then try it out (do), evaluate how well it worked (study), and make any needed changes (act). This approach allows for ongoing improvement and helps prevent gaps in communication. Improving communication not only enhances workflow efficiency, but also strengthens teamwork among staff. Ultimately, this leads to safer, more consistent care for patients. "A culture where reporting is encouraged, and system improvements are made is needed to enhance the quality and safety provided to patients. Practice settings need to develop this culture and become role models." (Gropelli, 2021, p. 288) As future nurses, being involved in CQI efforts helps us become more proactive in solving problems. It also empowers us to take ownership of the care we provide and continuously strive for better outcomes.
