You work for a local community service board from the assertive community treatment team as a mental health case manager. You have a client that you did an intake for. Justine a 22 year old trans male to female client. Justine struggles with heroine addiction, is homeless, has HIV and hepatitis C. Justine has a diagnosis of schizoaffective disorder. Justine often hears voices. Justine was beat as a child by his grandpa and emotionally abused by grandpa and parents the whole time growing up. Justine is now struggling with depression and passive suicidal ideations. Justine regrets their transformation and now wants to go back. Justine has been hospitalized 17 times the past 12 months. Justine is noncompliant with medication management once discharged from the hospital. Justine does not follow up on mental health care or physical health care. Justine does not follow through and get substance abuse treatment after discharge.
