Case Study

Tom Bart, a 67-year-old male client, returns from the cardiac catheterization lab after a cardiac angiogram. The results on the chart revealed a 95% blockage in the left anterior descending (LAD) and 99% blockage in the right coronary artery (RCA). The client will remain in the hospital and will meet with a thoracic surgeon about a coronary artery bypass graft operation to revascularize the heart. The client has an allergy to collagen, so the Angio-Seal collagen plug was not used. The client has a pressure dressing and a 5-lb sandbag to his right femoral artery in place. The LPN/LVN assesses the vital signs, noting T, 98.6° F; BP, 110/70 mm Hg; HR, 80 beats/minute; and RR, 20 breaths/minute. The dressing at the right groin site is dry and intact, and there are no signs of hematoma, bleeding, or edema. The pulses (femoral, dorsalis pedis, and posterior tibia) are clearly marked and all at +2 and the same as the baseline before the procedure. The client has normal sensation to his extremities and no pain, discomfort, or shortness of breath. The nurse reviews nursing management for the client and provides ongoing care.
