Question

Terry was a 50yo woman who presented with severe abdominal pain and was subsequently Dx with advanced pancreatic cancer. After her diagnosis she moved closer to her parental home for support. After several months, terry was referred to a home palliative care program for pain and Sx management after being unable to eat or drink; she stated she felt pressure or fullness at her abdomen. Terry, reported 10/10 pain in her RUQ of her abdomen, which radiated through her abdomen. She also complained of banding bilaterally from her umbilicus around to her spine. She had been on oxycodone 30mg q 3hrs, but with little relief. She stated she was desperate to have her pain managed. She also had constipation from the medications and was quite fearful of worsening pain. Terry was the youngest of 5 children. She was a binge drinker from age 17-33/ she lived out of state and was a house cleaner until her Dx. After her Dx she lived with her elderly parents with siblings nearby. She had difficulty understanding the gravity of her prognosis. Her 2 sisters were nurses and helped with her care.
