Hillary is a 42-year-old woman who was admitted for a total abdominal hysterectomy. Pain level is 6/10 pre-medication. Her temp is 100.1 orally, Pulse is 89 and regular, Respirations 18. BP is 122/78 and spO2 is 99% on RA. Her lungs are clear in all lobes. Heart is regular. Abdomen is flat, BS+ x4, tenderness noted around midline incision. Dressing change completed: Incision is midline, 10 staples, with small area of purulent drainage at the bottom staple. Incision is warm and pink around the edges near the bottom. States that she is upset that the Dr. had to remove her ovaries and she is not ready for menopause.
