He now requires closer monitoring every 2 hours. He is NIL by mouth (NBM), has IVT via a PICC line and an IDC requiring 2/24hrly measures. In addition, George has oxygen therapy via CPAP mask to assist his breathing. You are asked by the RN to monitor George's vital signs throughout your shift and maintain an accurate fluid balance chart.
2. You enter George's room and immediately notice that he is slumped in the bed. He is responsive to pain; his GCS is 8/15. His lips appear bluish and the CPAP is no longer on George's face.
