I need another sample of a case study about a Pt with Cystic Fibrosis. (Must be any age from newborn to 10 years old)
This case review consists of an adult male brought to the hospital. The patient is a 54-year-old, weighing 150 lbs. (68 kg). He arrived at his scheduled appointment on Monday, May 11th with a cough. Physical exam revealed he was sitting on the side of the bed with no distress. He has COPD. Assessment Upon physical assessment of the patient the following was observed: HR 82, RR 14, BP 124/86, SpO2 98% with temperature 100.3 F. Upon auscultation, the patient had coarse rhonchi throughout lung fields. When patient was asked to cough it was strong and produced moderate amount of thick yellow secretions. He said he had been coughing up for two days. He had no signs of cyanosis, tracheal deviation or digital clubbing, His skin was warm to the touch. Palpation of the upper lobes revealed tactile fremitus. Chest configuration was round like a barrel. Patient stated he had run out of his Combivent inhaler due to no refills. He stated he felt better after starting his inhaler regimen but was not sure why he was coughing up again. His grandchildren recently visited that just started school. He quit smoking ten years ago. The physician ordered a refill of the Combivent for maintenance of COPD symptoms. In addition, he directed the patient to take Tylenol, capsules, every six hours for fever. If symptoms persist for a week, he is to call in and an order will be placed for a chest radiograph. The patient understood the instructions and went home. Conclusion The patient appears to be experiencing a COPD episode due to cough, increase in sputum production, color and amount of sputum, breath sounds and fever. In addition, the patient stated he had a visit from small children. This could indicate a cold or viral infection since he has a weakened immune system (Nicholson, 2015). I agree with the management. The symptoms are common among COPD patients and it is best to monitor. If the symptoms progress this could be a sign of pneumonia development and a chest radiograph can be done for confirmation, followed by anti-microbial therapy (Nicholson, 2015).
Review - A. Alvarez (COPD scenario) Don't forget to Good explanation of the patient assessment.
Review - B. Bello (Asthma scenario) You mentioned the patient was a smoker. Is he still smoking? It would be best to clarify. It is interesting you mention the patient was visited by small children. This is a good indication that he probably caught a cold.
