Question

How to close and incident report about Patient started Rituximab infusion at 1800 - pt given prophylaxis benadryl, methylprednisolone, and tylenol 30 mins before infusion initiated. Patient observed in room for 15mins at initiation by nurse. Vitals stable, no reaction. Infusion increased at 30min mark - vitals stable, no reaction. Nurse came to bedside 10mins after increasing transfusion to find patient with chills, SOB, chest tightness, dizziness and nausea. Nurse stopped transfusion, flushed PICC line. Pulm team called to bedside. Patient placed on 5L NC to help with SOB. Patient still feeling extremely SOB, unable to catch breath - rapid reponse called. Rainbow labs collected on patient, EKG completed. Chest xray completed. Pt given NEB treatment and put on nonrebreather. IV benadryl 50mg given by nurse. Patient started to feel better. No longer feeling previous symptoms. Night shift nurse at bedside during code with day nurse. Full report given to night shift. Rituximab discontinued. All follow up orders, medications and labs to be completed by night shift. Patient now resting comfortably in the bed. Pending lab results.
