A community organization (nonprofit) went bankrupt.During the deterioration of the $10 million dollar organization the following issues were disclosed:

1. A lack of reporting on the uses and impacts of grant/donor funds that were designated for a purpose/program.

2. Poor accounting of how dollars were spent and co-mingling of funds that were for different purposes.

3. Personnel practices that did not include accountabilities and staff who were unmonitored.

4. Unpaid employee benefits which resulted in cancellation of health insurance even though employees still had deductions coming from their paychecks and were not informed that coverage was not in place.

5. Board members selling property for cash prior to the official filing of bankruptcy with no tracking of how that cash was used.
