"The Action Cycle links three activities-Planning, Implementation, and Evaluation. Each of these activities builds upon the others in a continuous and interactive manner. While the Action Cycle is the final phase of MAPP, it is by no means the "end" of the process. During this phase, the efforts of the previous phases begin to produce results, as the local public health system develops and implements an action plan for addressing priority goals and objectives. This is also one of the most challenging phases, as it may be difficult to sustain the process and continue implementation over time." (NACCHO, 2012) The instructor will provide specific instructions on how the Action Cycle should be organized and formatted.

The student will provide:

  • A description of potential public health partners and their potential roles in implementing the strategy
  • Using data from the four MAPP assessments, identify opportunities for or potential threats to sustainability that need to be addressed to assure successful implementation of the plan
  • A description of the requirements, methods, and participants involved in monitoring progress (e.g. Will there be a new surveillance system set up? A self-reporting advisory board?) Include justification for the method based on best practice examples from practice or the literature
  • Identify specific measures (i.e. data) that you will use to monitor progress
  • Provide a brief description of how successful achievement of health improvement will be measured (i.e. a realistic definition of "success" for the first five year period). This should include potential threats to successful measurement of achievement.
