Q. Locating the Problem in diabetes mellitus?

After having good rapport with the client and the family; you will try to collect the information about the patient, family and help in locating the problem. Start with general information about the patient on topics which are easy to talk about. This will help the client to gain confidence and give time to develop trust.

Slowly, go further and talk about family, economic status, support from family, health problems and go deep into them by probing further to find out the problem/s. You may keep in mind the following aspects.

· Open up the patient with general talk.
· Initiate topic easy to talk.
· Ask simple questions easily understood.
· Ask one question at a time.
· Later take detailed history.
· Do not interfere in-between.
· Do not suppress feelings, ideas.
· Make client free to talk and express.
· Explore the client's beliefs, values and respect them.
· Be an active listener.
· Repeat what client has said.
· Help the client to focus on the problem.
· Recognize the related problems.
· Make short notes.
· Make a summary of the talk and identify the problem.
· Assess the impact of the problem on the client's life.
· Explore the resources, coping skills and support available to the client.
· Explore solutions to the problem and discuss advantages/disadvantages/feasibility.
· Develop client coping skills.
· Motivate for change in life style.
