A one page website where a user can input the information including

Company Name, Title, Activity, R/S,

I also need the user to select the start date, end date, and check points, preferable from a drop down menu for each activity.

circle = start date

diamond = check point

Rectangle = duration

Triangle = End point

Master Plan Template Excel

I want this to be a single page website.

The entire template does NOT have to be visible on the website, only the PDF.

The left side of the template adding all text needs to be editable in the website.

The right side using the symbols should be configured as explained below:

Again, the graphics do NOT need to be visible on the website, only the PDF output which is standard size of A3 or 11"x17"
