Part of your well visit screening includes the Depression PHQ9 screening. Your 15-year-old patient Calvin has a total score of 16. In discussing the questions, Calvin reports that he has all Fs in his classes. He was not able to join the football or basketball teams due to his grades. He has been restricted from going out after school due to his grades. His parents are not home until close to 10pm. He is tired after school and naps from about 4 to 7 then has a hard time falling asleep. He has not been eating as he does not have an appetite. He thinks he would be better off dead. He denies any use of alcohol or drugs. What would be appropriate in this patient's care?
