For this extra credit opportunity, you will need to interview someone who works with the assessment or treatment of individuals with a chemical or behavioral addiction. If you are some who works in this field you will need to interview someone else other than yourself.

You will interview the person based on the questions below, and any others you think are important. You will than write a reflection based on the interview. You will turn in your interview sheet (the questions you asked, and answers you received) and then on the next page you will write a reflection about the experience interviewing this person. It should at least be a page long (double spaced is fine).
