Problem

A 17-year-old male patient was transferred from a small, rural hospital with a diagnosis of Guillain-Barré syndrome (GBS) (made on the basis of progressive weakness of both upper and lower limbs, autonomic dysfunction, cerebrospinal fluid analysis, and nerve conduction study). His breathing difficulty increased to the point that mechanical ventilation was initiated. The patient received intravenous immunoglobulin and volume control ventilation with other supportive measures. Three weeks after admission, the patient was showing signs of recovery (spontaneous breathing parameters: V.e 5.2 L/min, f 14 breaths/min, Vt 371 mL, VC 1.8 L, MIP -28 cm H2O; PBW 160 lb). After 24 days of ventilation, T-piece weaning (with a heated, large volume nebulizer) was used with increasing periods of spontaneous breathing.
