Case Study: Mr. X
You are a home health nurse visiting Mr X, a 63 year old Caucasian man, to assess and dress a chronic left diabetic foot ulcer. Mr. X has a past medical history that includes Hypertension (HTN), angina pectoris, Type 2 Diabetes and gout. His medications include Metoprolol, Ramipril, Diltiazem, enteric-coated ASA, Hydrochlorothiazide, Allopurinol, Metformin and Nitroglycerin spray SL prn. He admits to missing his medications often due to "forgetfulness". He is quite sedentary as the result of the foot ulcer and has a BMI is 35. He smokes 1 pack per day and admits to a poor diet, "ordering in" at least 5 times per week.
During your visit, Mr. X hobbles down to the washroom and when he returns, tells you he is having some chest discomfort and shortness of breath. He states "it's no big deal, this happens all the time". You do his vitals and they are BP 155/95, pulse 105 bpm, respirations 22.
