Question

A patient arrives in the emergency department with severe upper abdominal pain and vomiting. Studies reveal that this patient has "acute pancreatitis." This is the fifth episode of pancreatic inflammation this patient has suffered this year.

1. Would it be important to elicit a substance use history in this patient? (We haven't talked in detail about substance use disorder this week. You may need to do a little research or look ahead to next week's material. Consider the effects of abuse of different substances. Is there one there a use disorder associated with pancreatitis?)

The patient is admitted to the hospital. He is kept NPO (nothing by mouth) and IV fluids are started. He needs something for pain.

2. Would the nurse question an order for morphine?

3. If the patient has tolerance for alcohol, will he have tolerance for opioids as well?
