Ways to identify potential problems, protocols to correct problems once identified, and plans to reduce mortality and morbidity problems in the future (improved antiseptic techniques, use of more advanced surgical techniques and machinery, better pre- and post-surgical nursing care, improved patient screening techniques, etc.), are discussed and implemented. This conference is an example of a PSO that has existed in hospitals for many years and is used to improve patient safety, is a real-time teaching tool, and is actually mandated by governmental and accrediting agencies use concepts of reliability science, expected failure rates, non-catastrophic and catastrophic processes, improvement actions, measuring action plan effectiveness, etc., to achieve reliable patient quality and safety.
