Each of you will work independently this week to read, analyze and write-up an assigned health care organization role for the Case Study with a focus on how health information exchange is needed for EHRs health data information to improve individual and population health outcomes.
Case Study Scenario
The Illinois Department of Public Health (IDPH) tracks the leading causes of death at various levels (City/Township, County, Regional and State). Based on your assigned roles you are asked to address how Health Information Exchange (HIEs) may improve/reduce the leading causes of death by having the ability to share information.

Required Case Study Assignment Format:
Each case write-up should be 1000 words (+/- 50 words). Answer the four questions about the case in the order in which they are presented below. This will give you an organized outline for your write-up. Be sure that your response substantively addresses the following four questions.
Case Study Required Format
Background/Introduction - Background and introduction to the case study.
Analysis - Responses to the assignment questions using section header's and supportive references.
Question 1: Identify and explain the architectural type of HIE addressed in the case study. (This is your position statement.) (WO3)
Question 2: Analyze key EHRs data/information based on your assigned role requires for successful data sharing to help improve/address the leading causes of death. (WO2)
Question 3: Explain the benefits of the HIE presented in the case study and how it may serve as an example across the health care continuum. (WO4)
Question 4: Propose and defend three recommendations to address the challenges and roadblocks for your assigned role associated with data sharing, interoperability and coordination of care. (WO1)
Conclusion - Concluding discussion and comments using references as needed.
